Increasing Diabetes Awareness throughout Georgia

A key component to transforming diabetes in Georgia is increasing the awareness of diabetes and the expectations of care along with increasing knowledge of prevention and delay of onset of the disease.

Working with major employers, providers and insurers in the state of Georgia through the Center’s Bridges to Excellence Initiative, the message on standards of care for individuals with diabetes is being delivered to providers, employers and individuals with diabetes.

Providers

The Center supports the standards of the NCQA Diabetes Physician Recognition Program. Through the DPRP assist program in Georgia, a program funded by Novo Nordisk to educate physicians on the NCQA diabetes recognition program and assist them with the recognition process, physicians in Georgia are being educated on the standards and receiving recognition.

Employers

Through the Center for Health Transformation’s pay for performance initiative, major employers in Georgia are collaborating to bring awareness and screening programs on diabetes to their employees. The Center also participates in employer meetings and Chamber of Commerce events to educate the employer community on the impact of diabetes

Diabetes National Websites
American Diabetes Association [ADA]

The ADA has a website filled with information on diabetes and a call center for those who need to talk to someone in person (1-800-DIABETES) (Information offered in English and Spanish)

The Juvenile Diabetes Research Foundation International [JDRFI]

JDRFI supports research aimed at finding a cure for type I diabetes. Their web site has a section on for children that provides help for dealing with diabetes day-to-day.

The National Diabetes Education Program [NDEP]

NDEP is a partnership of CDC, NIH and over 200 public and private organizations and is a federally-sponsored initiative that involves public and private partners to improve the treatment and outcomes for people with diabetes, to promote early diagnosis, and to prevent the onset of diabetes. The Center for Disease Control & Prevention [CDC]

The CDC has a diabetes division that translates diabetes research into daily practice. The CDC publishes fact sheets and other useful data about diabetes.

The National Institute of Diabetes and Digestive and Kidney Disease [NIDDK]

NIDDK offers the National Diabetes Information Clearinghouse which provides information for people with diabetes, including information on clinical trials. (Information offered in English and Spanish)

A part of the National Library of Medicine, MedlinePlus has extensive information from the NIH and other trusted sources on diabetes and other diseases. (Information offered in English and Spanish)
