The Georgia Project: A Summary Report on the Center for Health Transformation Pay for Performance Initiative

The Center for Health Transformation brought together a coalition of employers, insurers and providers in partnership with Bridges to Excellence (BTE), a non-profit dedicated to improving healthcare quality through implementation of pay for performance programs, to launch what turned into a three and a half year project to improve the care of individuals with diabetes in Georgia. This paper is a summary of the lessons learn and outcomes achieved from the project.
